There are no requirements for filling out the report below.  Those filled out completely will allow us to better understand and learn from your experiences.  Incomplete reports may be delayed in getting posted or not used at all.

We accept current and historic ghost reports. If you have an older report and do not remember the exact date, please give us as much as you do remember.
